This is what I currently have! Should I stop using this?!
Yes, I would stop using this because it has D3. Since you have a calcium WITH D3, you can use it 2 times a month, every other week, and alternate it between a multivitamin with NO D3. So one time one week(like mondays) you would use this calcium, then the next monday you would use the multi with NO D3, then the next monday use this calcium with D3, then the next monday use the multi with no D3...and every other day you feed you will use a calcium with no D3.

I just like to make it easy on myself. So I use a calcium with no D3, except on the 1st and 15th I use reptivite multivitamin WITH D3.

@Beman would be the person to ask about different bulbs and distances they need to be from your cham.

I would buy a T5 if you can find them.
You can buy a regular t5 fixture (like for growing plants) and then buy a 5.0 uv bulb to put in it.
 
Can you post a few more photos of him from today or recently so I can see all of both sides of him please.

I see @kinyonga is here. She is incredibly knowledgeable about chameleons!! We are lucky to have her in the chameleon world.

Honestly, not very often at all. Every now and then. When I sprinkled it on the crickets, they died very fast so I probably wasn't putting enough anymore :(
 
The pictures provided are of his smaller terrarium that he is currently in, the large cage he was in, and photos of him through-out this time when I started noticing odd behavior.

try Pangea.com for the lights. You’re going to have to get the fixture and uvb separate. They have both reptisun t5 ho 5.0 and they also have the Arcadia 6%. Either will Work. I’ll attach links

also make sure his branches are 9 inches below his lights

So, just so you are confident about supplements, I want to make sure you know...
When you are dusting the insects it is RIGHT BEFORE they are put into the feeder run, or dish, or enclosure....however you feed your chameleon. You lightly sprinkle a little supplement on the insects and then feed them off immediately.

I am currently having to force feed him. Any tips or recommendations on that?

Today when I went to the vet, they gave him liquid metronidazole 50mg (1 mL) and invermectin (an antiobiotic and meds to treat parasites). Will this negatively effect anything if I go tomorrow and ask for treatment for mbd? They shouldn't counteract, right?
 
Also, is mbd something that I will have to continue treating for the rest of his life? Or will this be a one time (ish) treatment and if I stay on top of the UVB lightening, feeding, and vitamins he should be okay for the time being?

He needs calcium and uvb the most at this point. I’m not as experienced with chameleon meds so unfortunately I have no clue, I know medicine can be hard on their body though.

With MBD I’m assuming a vet will give him calcium shots. Probably a few in his case, but not all at once. And on top of that you will need to stay on top of his supplement schedule. And make sure he has correct uvb lighting. Supplementing and uvb go hand and hand.

I’ve never had to force feed before so I’m not the person to ask, but MBD can be stabilized I guess you could say. His limbs are very bowed but if his grip is still there I’m sure he will have a chance. You just need to do research

They have a tro kit with the earth pro A (calcium with no d3) calcium pro Mg (2x month), and revitalizeD3 (2x month).
