What is going on with him? Will this heal? Do I need to take him to the vet? Thank you!

I would take him to the vet, that's a considerable hole. Was there a lump there before, or is it an injury?

It's an injury :/ he fell.... And hit his face and then it started crushing over and then fell again and the crust fell off... Is there any ointment I can try before paying 90$ for a consultation?

Looks like it has developed an abscess . It will need to be cleaned out and he will need antibiotics. Plus, he shouldn't be falling... definitely time for a vet visit.

The problem with waiting to take him to the vet and attempting to treat it on your own, is that it could get a lot worse. Then he'll be in worse shape with a much lower possibility of a good outcome. The way it's swollen at the base of the wound, makes me worry that infection is already present.

Also, I would try to figure out why he's falling. Like Lathis said, he shouldn't be falling. There's usually a reason behind it. Like a foot/nail injury, some sort of vitamin/calcium deficiency, or poor cage layout. I would read over the care sheets here, and see if there's anything you need to correct. I would also find a vet that's experienced in treating chameleons to take him to.

He likes to climb so we let him walk around the house sometimes.... And he sometimes tries to climb things that he can't like chairs and ends up slipping and falling :/ but I guess I really should take him to the vet

Also the pet store guy said that he may be rubbing his face on the glass cage.... Which I've also seen him do

This looks like a classic snout rub to me. To keep it from getting infected and help it heal faster, try dabbing Polysporin (no pain relieving ingredients) on it and watch for more rubbing.

I would make sure that if you're taking him out that you have a suitable free range plant or area that you let him roam in, and not on household things that he could slip on. Just my two cents here.
