I read this whole thread almost knowing that he would probably not make it but i had a small beacon of light in the back of my head D:
Seeing that picture and now reading that he is gone seriously almost made me cry!
i have a weak spot for sad things!

Oh my! The poor little thing looks really bad!! He might fell? That would probably explain the cut and the abrasions..
Can I ask you why he was not getting any supplementation? I'm not totally sure about it but a staple diet should be provided by crickets, and certainly I would remove any soil from the bottom of the enclosure. Last not least I would place him in a screen cage, as fish tanks are not exactly ideal for chameleons.
I'm hoping in some miracle for your little guy.:-(
Please make sure you go though all the informations this amazing forum can possibly give you to make sure you'll definitely know everything in case you'll decide to get another Cham.. I'm very sorry!:-(
