Chameleon Info:
Your Chameleon - The species, sex, and age of your chameleon. How long has it been in your care? Male adult Jackson, approx 1 year old, I have had hime since April 2012.
Handling - How often do you handle your chameleon? Weekly to move for cleaning, take out on nice days.
Feeding - What are you feeding your cham? What amount? What is the schedule? How are you gut-loading your feeders? crickets, dubia roaches, super worms, blue bottle flies, tried silk, horns and butter worms. Was feeding approx 5/6 insects a day in the morning mostly. Did give a few wild caught flies,moths,bees occassionally. Gut load with a dry formula that I bought from a cricket breeder and also assorted high quality fruits and veggies.
Supplements - What brand and type of calcium and vitamin products are you dusting your feeders with and what is the schedule? Repashy Calcium Plus, one pinch a week.
Watering - What kind of watering technique do you use? How often and how long to you mist? Do you see your chameleon drinking? One large dripper a day, mist by hand twice a day, and a humidifier 5 times a day for 30 minutes (on a timer) was drinking recently, has been a few days now since I saw hime drink.
Fecal Description - Briefly note colors and consistency from recent droppings. Has this chameleon ever been tested for parasites? Not much lately. They have been longer then rounder like they used to be. Small and brown in color. Urates are mostly white with a little orange/yellow tint to them.
History - Any previous information about your cham that might be useful to others when trying to help you. Bought him at a reptile expo in Indianapolis. Dont know his birthday.
Cage Info:
Cage Type - Describe your cage (Glass, Screen, Combo?) What are the dimensions? Screen 18x36
Lighting - What brand, model, and types of lighting are you using? What is your daily lighting schedule? 5.0 long tube UVB 7:30 to 7:30. Spot light is two 25 watt bulbs on a same schedule in a strip light under a branch. One side is closer to bulb and is at 88 degrees other side is lower and at 83 degrees.
Temperature - What temp range have you created (cage floor to basking spot)? Lowest overnight temp? How do you measure these temps? 88 top front corner, 75 in the room, not sure of temp under the plants. low temp is around 72 at night. measure temps with reptile thermometer.
Humidity - What are your humidity levels? How are you creating and maintaining these levels? What do you use to measure humidity? 50 to 80 % using a dripper, humidifier, and misting. measure with a reptile guage.
Plants - Are you using live plants? If so, what kind? 2 large Pothos, one up and one down.
Placement - Where is your cage located? Is it near any fans, air vents, or high traffic areas? At what height is the top of the cage relative to your room floor? In my office, one cieiling fan, not near ac/heat vent. Sitting on a 3 foot shelf, top of cage is approx 6 feet at the top.

Is not eating, very lathargic, hardly moving. Has a funny looking lip. Eyes are closed, sleeping during the day. Just gave him a 30 minute shower, taking him to the vet Friday, first day I could get in.
