Looks like an abscess from a damaged toe nail. Your vet needs to lance it and clean it out. It isn't a big deal for the vet to do. My vet has done it in an office visit with no anesthetics. Double check with the vet on how they would do it and the cost--there are some vets, especially exotic vets--who seem to think that they have an obligation to make things as expensive as possible.
