Chameleon Info:

Feeding - What are you feeding your cham? What amount? What is the schedule? How are you gut-loading your feeders? I offer him 10 crickets and 5 meal worms or wax worms a day

Supplements - What brand and type of calcium and vitamin products are you dusting your feeders with and what is the schedule? I use rep cal with 0 d3 once a month and rept life plus once a year
You should be using the plain calcium almost every feeding, in extremely light amounts. The feeders have a cal/phos ratio of 1:2. For proper calcium absorbtion, you need a ratio of 2:1. Thus, dusting with an EXTREMELY light amount of calcium all the time/almost all the time is the best way to get the animal what it needs.
Watering - What kind of watering technique do you use? How often and how long to you mist? Do you see your chameleon drinking? I spray him in the morning afternoon and just before bed and I have a fogger on him
For how long do you spray him?
He needs at least four mistings a day. Of around 5 min each, and one of them should be around 10 min long.
You want the cage to dry out in between mistings, but you especially want it to dry out before night. This will prevent mold and mildew from growing in your encosure, which can make the chameleon sick.
A fogger should not be used all the time, unless you live in an area where the RH levels are extremely low(<10%), for the same reason as stated above.
Fecal Description - Briefly note colors and consistency from recent droppings. Has this chameleon ever been tested for parasites? Dark brown with like cricket remains in it and no he has never been tested for parasites
Undigested feeder parts should be a rarity. Is there any yellow, orange, or white to the droppings?
There should be a white part, which is the urate. If it is yellow or orange, then the animal is dehydrated, and not getting enough water.

Cage Info: many branches and lots of foliage
Cage Type - Describe your cage (Glass, Screen, Combo?) What are the dimensions? All screen cage with a foam rock back ground and I am pretty sure it's 24 by 24 by 48
Lighting - What brand, model, and types of lighting are you using? What is your daily lighting schedule? I use a zilla UVB with a zilla rainforest UVB (5.0) and a 50 wat threat bulb

You need one 5.0 UVB bulb, and one 40-75watt heat bulb.(depending on your situation)
Temperature - What temp range have you created (cage floor to basking spot)? Lowest overnight temp? How do you measure these temps? It is around 81 degrees and around 60 degrees at night
Humidity - What are your humidity levels? How are you creating and maintaining these levels? What do you use to measure humidity? 80 to 90%
Plants - Are you using live plants? If so, what kind? I don't use live plants in his cage but I used to about 3 months ago it was a real bamboo tree/plant
Some bamboo is toxic, though I doubt he would eat any of it anyhow. You really need live plants in the enclosure. THis will generate fresher air for the animal, and keep your RH levels up.
Placement - Where is your cage located? Is it near any fans, air vents, or high traffic areas? At what height is the top of the cage relative to your room floor? The cage is near the entrance of my door and there is a fan in the middle of my room witch is only on at night

Current Problem - The current problem you at having. He seems to have a loose grip and sleeps during the day plus he ignores food unles it's crickets that are hand fed to him
