Chameleon Info:
- Your Chameleon - The species, sex, and age of your chameleon. How long has it been in your care?
- Handling - How often do you handle your chameleon?
- Feeding - What are you feeding your cham? What amount? What is the schedule? How are you gut-loading your feeders?
- Supplements - What brand and type of calcium and vitamin products are you dusting your feeders with and what is the schedule?
- Watering - What kind of watering technique do you use? How often and how long to you mist? Do you see your chameleon drinking?
- Fecal Description - Briefly note colors and consistency from recent droppings. Has this chameleon ever been tested for parasites?
- History - Any previous information about your cham that might be useful to others when trying to help you.
Cage Info:
- Cage Type - Describe your cage (Glass, Screen, Combo?) What are the dimensions?
- Lighting - What brand, model, and types of lighting are you using? What is your daily lighting schedule?
- Temperature - What temp range have you created (cage floor to basking spot)? Lowest overnight temp? How do you measure these temps?
- Humidity - What are your humidity levels? How are you creating and maintaining these levels? What do you use to measure humidity?
- Plants - Are you using live plants? If so, what kind?
- Placement - Where is your cage located? Is it near any fans, air vents, or high traffic areas? At what height is the top of the cage relative to your room floor?
- Location - Where are you geographically located?
Current Problem - The current problem you are concerned about.

my chameleon - marley, they said she was 2 but vet thinks a little bit younger, somewhere around 1 & 1/2
• handling - recently, had to switch enclosures & go to vet all other times only when needed
• feeding - crickets, roaches, gut loaded orange cube cricket diet. i feed her as much as she’ll eat right now because she needs to put on some weight.
• supplements - most days dusted w/o D3, with d3 2 times a month & multivitamin 2 times a month opposite weeks w D3
• watering - monsoon solo mister, during the day 20 seconds every 2 hours, night time everything’s shut off - as recommended from a jackson’s breader - as well as little dripper dripping on leafs 24/7
• fecal - haven’t seen any in a couple days, need to get tested
• history - was at petco for 8 months, just got her a week ago. she came very skinny and sick, since then we’ve made a lot of progress, i think it was over supplementation of the D3 and what just comes with being improperly cared for for 8 months
• cage - medium size reptibreeze
• lighting - t5 linear uvb, 75 watt basking. no light at night. 13 hour schedule, lights come on at 7am, go off at 8pm
• temp - bottom of the tank corner coldest gets about 63, basking can get up to 90 (trying to work with basking atm) overall cage is around 73 - 80
• not measuring humidity (as advised by jackson’s breader, a daily schedule should be good at 20 seconds every 2 hours off at night)
• plants - golden schefflera (arbicola) and gold pothos. both plants were re potted in coconut substrate as well as rocks are covering exposed substrate
• placement - on my desk about 3 feet away from the window, top of the tank sits just under 5 feet from the floor
