Health Record Ideas

Hello everyone! This is my first post here and I am interested to know how everyone keeps their health records? I recently put this short page together gathering the basics but I would love input on what I'm missing or different ideas....thanks!!

Nice form, good for record keeping.
I would suggest for the female only section Bred to: or Sired by: for the breedings.
In the meds section the spaces for meds given and diagnosis will probably need to be longer.

Great ideas! I only have one male at the moment so the "sired by" didn't even cross my mind but I'm definitely going to add it just in case ;)
 
What's this for? Seems like the kind of thing obsessive list makers like myself would do, only to cause unnecessary stress :rolleyes:
While I am a self-aware obsessive list maker I am also however, the daughter of two veterinarians....and if there's one thing I've learned from working with them for years it's the incredible importance of keeping a detailed health record. Animals can't tell us what's wrong with them when they're sick so if we can have as many clues as possible (such as a weekly weight or a good record of clutch history), then we might be able to get to a diagnosis quicker! So it might be slightly obsessive of me but it at least gives me some peace of mind :D:D
 
As a former RVT, AHT or LVT depending on the state you are from, I agree with good record keeping being an important part of husbandry. Chameleons, just like birds, do their best to hide their illnesses so they won't appear vulnerable. Having a known baseline for comparison is a valuable tool.
I thought of one more piece of info you could use as a measure. For your juveniles tracking their body length monthly might be useful. I have mine climb on a wooden ruler to get an approximation of body length.
 
I wish I could be this obsessed with list. I just jot all my stuff in a notebook... which is an awful mess for anyone else to read.
