Chameleon Info:
•Your Chameleon - The species, sex, and age of your chameleon. How long has it been in your care? Max is a veiled chameleon, 9 months old, I have had him for about 6 months
• Handling - How often do you handle your chameleon? Max hates to be touched so I do not handle him. I am afraid of stressing him out.
• Feeding - What are you feeding your cham? What amount? What is the schedule? How are you gut-loading your feeders? I am feeding Max 5 crickets a day and 2 super worms. The crickets get Fluckers High Calcium Diet, and Fluckers Cricket Quencher Calcium Fortified. Max will not eat any vegetables.
• Supplements - What brand and type of calcium and vitamin products are you dusting your feeders with and what is the schedule? Crickets are dusted before I feed him and they are dusted with ReptoCal
• Watering - What kind of watering technique do you use? How often and how long to you mist? Do you see your chameleon drinking? Max has a water fountain in his cage and I change the water once a week. I see him drinking out of it daily.
• Fecal Description - Briefly note colors and consistency from recent droppings. Has this chameleon ever been tested for parasites? His droppings are mostly dark in color but sometimes it can be white. I have never seen any parasites.

Cage Info:
• Cage Type - Describe your cage (Glass, Screen, Combo?) What are the dimensions? Cage is glass, 30x12x12
• Lighting - What brand, model, and types of lighting are you using? What is your daily lighting schedule? Don’t know the brand or model but he has a UV light which is on 9 am to 9 pm and he has a heat light that is on 24 hours a day.
• Temperature - What temp range have you created (cage floor to basking spot)? Lowest overnight temp? How do you measure these temps? I have a thermometer in his cage and it stays pretty much at 75 degrees.
• Humidity - What are your humidity levels? How are you creating and maintaining these levels? What do you use to measure humidity? I have a humidity gage in his cage, it reads 75-80 most of time. His water fountain and misting is what creates the humidity.
• Plants - Are you using live plants? If so, what kind? I use both artificial plants and live.
• Placement - Where is your cage located? Is it near any fans, air vents, or high traffic areas? At what height is the top of the cage relative to your room floor? Max’s cage is not near any fans, air vents or high traffic areas.
