It sounds like parasites to me, you mentioned he's never been tested. I've seen chams come in with round worms that would sit with their eyes closed during the day. After panacur treatment they were fine.

My free range room can get cool too, so I have a space heater plugged into an appliance timer so it only comes on during the day to eke the room in the low-mid 70's. This way the temp can cool down at night.

I also would recommend you go with a bare bottom. Some are successful with the soil substrate but for a new Cham owner, it can spell disaster. Mold, impaction from eating it, flies, and drainage can all lead to trouble. The shamwow is an awesome idea.
