The vet wasn't much help

I went to the vet today and it didn't go so well thy just recommended another vet that was more expensive and further and that he might surgery but he won't make it because his so small so I just want a second opinion
Dose any body have any idea what's the thing around his eye? And how to fix it whit out surgery

What kind of light blubs are you using for UV and heat?

I was told to Chang the UVB from a coil to a tube and for heat I have a Zoo Med Daylight Blue Incandescent Reptile Bulb 60 Watts
