My chameleon is passive (and walk strang)

Hello.
My chameleon is 5 months old , meal,
I noticed yesterday and today
when he opened his mouth i saw black teeths
and is so passive walking too rare and he is shaking when walk

Please help me!
i am so scared (((

Sex- meal
age- 5 mouths
care - 2 months
feeding - Superworms, and some weeks ago i feeded withe Mealworms
Supplements - no using cus i dont have where to buy in my country , i order from ebay D3 calcium but steel waiting
Watering - with syringe without needle, i am very careful , he drink every day
Fecal Description - i noticed blue points
History - 2 month ago when i just brought he broked his tail when he tried to open door, but know is oky the black part fall and looks like is growing again green but not sure

Cage Type- glass and on top is plase metalice for UVB
Lighting
- one 12W 5.0 UVB and other for warm 75W UVB 3.0
Temperature - 25 degrees at night, 29-31 degrees at day time
Humidity - try to be all time 40%-60%#
Plants - not using
Placement - nothing is near on table near the wall
Location - Moldova
