Your basking temp may be a bit high for him, how are you measuring the temps?
How close is the basking spot from the light bulb?

Are you referring to viseral or articular gout?

Articular gout is easy to see, as it distorts the limbs and swells the bone joints.
Visceral gout can not be seen, as it is only inside the gut. But the warning signs are as follows: slow, clumsy moving, lack of interest in drinking water, dangling limbs, lack of strenth.
Visceral gout is often not detected until it is too late, and a diagnosis of this desease is usually a death sentance.
Only a blood test to check uric acid can determine if he does indeed have viseral gout. You would need to take him to a vet and have him/her take a blood sample.
One of my panthers died from it.

You are not giving enough info.
Why do you think he has gout?
Are his urate constantly orange or red?

Okay, I see now.
Is it still just the one foot/leg with the swelling?

Gout would not just effect one limb, you would see it in other areas as well.

Reptiles take a long time to heal. I would not worry about gout just yet, and make sure he is well fed with suppliments and plenty of water.

I would also move him to a cage where the possibility of falling is low.
May put something on the floor to cushion any fall.

Is he licking the swollen area, and trying not to use it much?
He may be in pain, but I see your post that the vet gave you some meds for that.

I would just wait and give it time.
