Leon has become thin and his eyes a sunkin. Please HELP!!

My chameleon is looking sick and I don't know what's wrong. Hes not gripping as hard as normal and has something black he's trying to get out of his mouth please let me know what could be wrong with him.

Please include a pic, usually a sign of dehydration includes the eyes sinking in, fill out the How to ask For Help sheet so we can better assist you, mouth rot could be a possibility, but we cant know for sure unless a pic and proper info is given. There are so many helpful people on here so someone will definitely be able to help.
