Does it move? I mean, it looks hydrated .. worms thrive on dampness. If it doesn't move, it might have come from one of the chams.
Just to be safe, look in the soil... you never know.

Is it possible that you could take each individual plant and repot them? I took each stem with the root and put each one in a pot and spread it out throughout the enclosure. That way it's not so tight together.
You would think it looks like an earthworm, but then why can you see peices of brown stuff inside it? Earthworms don't look like that.
Can you take the worm-thing and mush it? See what's inside it?
You might want to take samples of all your chams poop and get a fecal done to be sure ....

Best substrate: organic soil or coco fibre .. make sure there are no clumps and comb through the substrate every so often
I use it with much success and many others have/do as well.
If you notice your cham eats it, take it out and see a vet. It may mean subsequent issues with your chams...
Wow, I've never heard of this happening .. I hope that your cham adapts with the loss. Just give your cham special attention. Hopefully that assists in a full recovery and your cham will be able to continue living a long and healthy life.

From the pics, especially the third one in, it looks like a peice missing from the casque. VERY small but you would know more by seeing him in front of you. It looks like a hole from the pictures. Could be nothing .. I just noticed it though.
Just with regards to reflection in plexi, or glass for that matter. Many people use glass enclosures and keep their chameleons in them successfully. I personally don't see my chameleon looking at the glass at her reflection at all. There's nothing wrong with plexi or glass.
He has some sort of hole on his casque .. is that something to worry about? The arms look a little 'fatty' in the middle as well. Other than that, he looks very cute! I'm looking for an adult male everywhere but no luck! Did you purchase him? Maybe a vet check up would be helpful.
