You did everything that you could and there was nothing that could have helped except eliminating his pain.. You did the right thing and there is no reason to beat yourself up. Im so sorry for your loss and just know he is at peace and his pain has ended..
Make sure to give him a couple extra mists to aid him along... Preferably with warm water as this helps them along... When he's done if he's tolerable to handling a nice trip outside REALLY helps their colors and helps them acclimate their new skin :)

It's excess minerals... Not necessarily due to over-supplementation but your right it's mostly salts and is usually pretty normal... Just keep an eye on it for a while in case anything else changes
