Buying from printshop question

Hello everyone im goign to buy a shirt from the print shop but cant figure how to get my user name on the shirt. Im ordering it today and i cant find where i can choose to have my name on it? Anyone know?

Another bump, Um i feel so bad for doing this but please guys any help i need to order it tonight but i dont know do i make the request after i put in the billing and shipping info? it says billing and shipping, place order, and then receipt.
