A potted plant is not adequate, you need play sand as a substrate, and the cham will have difficulty digging through the roots... Even if it could, then you are harming your plant.
Can you fill out the whole form with the current temps and humidity specified? What you think is recommended might not be right... We can't help you if you answer each question saying it is what's recommended...

Oh, my bad, I didn't read your post fully. From the first one she is definitely too small to be sold, but in the second one it is hard to tell, since there is no reference for size. Can you post more current pics of her?
On another picture you posted in the thread "Shedding", his eyes did not look sunken in. Something is causing his eyes to be sunken in, wether it be stress or dehydration.
You should throw away what you have now pretty much. You need a new uvb bulb and fixture. If you would like to keep the dual dome with just the heat bulb in then you can also do that.
Yeah, I do. @Kaizen must have forgot to mention in his feedback that you need a linear t5 ho linear bulb. The dual dome you have will kill your cham.
