Definitely stop using both Calcium Plus LoD and Vitamin A Plus at the same time. Both contain Retinol Vitamin A which is preformed Vitamin A. I would just cut out the vitamin a plus and only use the LoD for every feeding.

You definitely want a T5HO fixture from either Lightyourreptiles.com, Arcadia, or ZooMed that spans the entire length or width or diagonally across the top of your enclosure. As stated on your other post it's important to have the correct fixture as well as bulb. Not all fixtures are created...
Problem with that is the ballast inside your Walmart fixture is not made for UVB bulbs. You're probably burning out the phosphorus in the bulbs way faster than intended this making your bulbs less effective in the long run.
