Moving. Help, please!

Hey, guys, me again.
I currently live in Hawaii, and I got my Jackson's cham Latoya, here. However, by a strange turn of events, I just got into art school in New Hampshire, and will be leaving for the mainland in a month or so.
I searched online without much luck, but does anyone know the procedures for transporting a herp from HI to the mainland? I assume it would qualify as US to US, but I had to fill out one of those US Customs plant and wildlife Declaration forms when I first got here, so I'm not entirely sure.
I looked on this site:

but I'm not clear on what-if any- of this actually applies.

Please, if anyone has done something like this before or has any advice, It would be exceedingly helpful.
